Freedom of Information Act (FOIA) Requests
Under FOIA, individuals have the right to inspect, copy, or receive copies of public records of a public body. If you would like to obtain records in which are public, please submit a request to the City Clerk's Office using our FOIA Request Form. All relevant documents can be viewed below:

Health Insurance Portability and Accountability (HIPAA) Requests
Certain records of a public body are considered protected under HIPAA. It is the City's responsibility to protect the privacy of individually identifiable health information contained within these documents. These records may be obtained in the event authorization is given to the requester from the individual who is subject in the protected record using the City's Health-Related Record Request Form - Other. These records may also be obtained if the individual subject in the protected record is the requester using the City's Health-Related Record Request Form - Individual.
