Eligibility for the QRT

Eligibility for the QRT

To meet the criteria for the QRT, an individual must live in the city limits" and be experiencing or have a history of at least two of the following OR have a history of experiencing overdose:

• Substance Use Disorder

• Mental Health Crisis

• Homelessness

This individual would be appropriate for the QRT if they need help coordinating wrap-around services, have had a recent overdose, and/or need to get connected to additional QRT partners/community providers.
